I am not sure if the Disc picture is coresponding to this album, as i don't know much this series.

It's probably correct. PAM-0028S is this disc which was released in 2004, while PAM-0047YS corresponds to the soundtrack re-release with extra tracks, which matches this entry. The catalogue number printed on the disc matches as well. The album title should be changed to reflect what's printed on the disc though, I'll do that.
