I took some pictures of the contents of the box for anyone interested. https://imgur.com/a/V5ert
It's supposed to be blue, red, and green marbled vinyl, but I can't say I see much green in the Dark Souls III records.
The tracklist on the box is incorrect, but luckily the individual sleeves have the correct one.

All in all this is a very nice product by Bamco, and even though the padding inside the box it came in wasn't as much as I'd prefer, given how big and heavy this item is, nothing inside was damaged in any way. The records had not even cut through the paper sleeves, which can sometimes happen during shipping.
Highly recommended if you're into Dark Souls.

Does this give proper composer credit to DS2's DLC/SotFS tracks? The original CD didn't really say who composed what (though Kitamura did confirm she did Sir Alonne's theme).

No, this release shows no composer credits whatsoever.
