Title is without "Ban / 盤" for this child album...

It should also be noted that on the left side of back cover title it is written オリジナルアニメビデオ アモン・サーガ「千の翼・千の夢」. 「千の翼・千の夢」can also be found on the obi and back cover. Same on LP obi.

Shouldn't it be a part of the title, then?

Yes. "オリジナルアニメビデオ" can be omitted from the display title though.
