The Booklet 2-3 scan has some gaps in the names in the credits, from what I'm guessing is a printing defect. So the first arranger isn't "Erika A__ya," but Eriko Akiya, confirmed on her website. Likewise, the mastering engineer Kazushi Ya__zak is missing some letters (Yamazaki?), as is Goro ___ozaki from ASCII.

Also, if you believe a few online retailers, Yuji Otokawa's name in Japanese is 音河 修二.
