The discography pages on ZIZZ's old site used to include some specific composer information that wasn't actually printed on the albums, so I expect the breakdown came from there because there's none on the album for the BGM. Unfortunately the page for this album isn't archived, so I have no idea if the credit to Ohyama came from there or is our own mistake.
In the absence of clarification on the last point, it seems safe to just change the credit to Izumi based on the digital distribution. JASRAC has Izumi down for that track as well.
