there are two front covers existing
one with (hidemoto) KEIKAIN aligned with YURA KEIKAIN (vgmdb one)
one with (hidemoto) KEIKAIN a bit more to the left with something after KEIKAIN (could be 13th, but without disc or tray or anything blocking the title like characters drawn it's not possible to know)
I think vgmdb one is the right one since it's on pony canyon site and only hidemoto keikain seems to be there without the 13 thing
so I removed the 13 thing and changed the place of character names (hidemoto/yura -> yura/hidemoto) as the cover shows
