Premium CD Drama part of the title is not listed on the covers, only on the disc.
GMR gives November 5th as the day of release, which may or may not be correct. The album itself states nothing further than a few instances of '©1993'.
edit: I'm not sure how to interpret the trackname for track 3. And
snesmusic.org backs up the release date and the idea that it was a bonus (first print only?) for the game.
