I got a copy of the game with the soundtrack enclosed. I added images: case (front, back), right spine (has the title written in Japanese), disc, insert (front, back), game box front, game box back (the detail where it says the soundtrack is enclosed, and has the price for the game), and a page from the game's instruction booklet with credits. The tray is just black plastic.
The insert is actually a folded piece of paper, but there's nothing printed on the inside, just white pages.
Also, on the back of the insert, tracks 1~3 are in red and 4~6 are in green. I'm guessing that is indicating that 1~3 are from BATTLE GODDESS and 4~6 are from MAID IN BUNNY.
