Gradius FC (track 04) doesn't have any credits, but I've put Maezawa based on
this interview:

HM: Gradius and Contra I worked on completely alone.
1UP: Those were the home versions of the arcade games, right?
HM: Sort of. For Gradius, I ported the music from the arcade version, but it also had original songs on Famicom.

In
his site, he only lists Gradius 2 FC (though he adds "many others"), but in the interview he is definitely must be refering to Gradius 1, because on Gradius 2 he worked with Yukie Morimoto (and Motoaki Furukawa helped with the ending) thus not alone. He is asked later about Gradius 2 FC too.
