I added images for this one: case (front, back), insert (front, back), and disc.
I changed the catalog number from PLJM-16238 (which is for the game, I think) to APCD-0036, as printed on the soundtrack disc.
I'm guessing that the catalog number for this album, which also starts with PLJM, is also incorrect:
