There's a mention of it on STC: "the 3-song disc that kMart gave with Ocarina of Time..."
Other than that, I dug around on freedbtest and
found this 4-track CD. There's a
VGMdb entry with apparently the same 4 tracks but with german titles, and it was a Europe promo disc.
