Could it be a special case where the hidden track is noted in the comments, but it's length is taken into account in the total CD length? Or would that cause some back-end trouble? I suggest that because including it in the actual track list might cause problems with the freedb feature (the track names would all be off by one). A special field / flag denoting CDs with hidden tracks could be handy too. Then people can tell at a glance if there's a track 0.

According to my knowledge the only way to rip the hidden track is to attach the gap to the following track (in this case track 1) and then use audio software to split the ripped WAV into two different files. Since it's so much work (and ripping programs can't do it automatically) it might be better to optimize for the general user. In other words, ensure that freedb can detect the CD and send the correct track list. Anyone who wants the bonus track can easily use the info in the comments to find and extract the hidden track.
