It's odd that there's hardly any information online about this release. I can't find a release date, the closest thing is this
blog post which seems to suggest it's a C75 release. It was also certainly never sold through doujin shops so I'm calling it 'event only' for now.
Edit: Actually, that's the blog of the guy who did the jacket design, so I'm going to go with it.
