I just received my copy from Kahori. The "album" version that she sells at VGMWorld is actually composed of two albums / cases. The normal case contains the OST and has catalog number CPCA-1089 and the slim case contains the bonus disc and has cat no. CPCA-1090T (and does indicate "NOT FOR SALE").
