As Efendija said, it is both. Originally it has the GTA IV attachment but it was also distributed on Liberty City Stories with Gay Tony, which didn't require the GTA IV original installation.
So it may help to distinguish which portion the game falls in (between III and V). On the other hand it does show up on the Covers and images all over the site, so I would say it probably is best to add the IV
