So it turns out there were actually 4 members on the sound team for Super Castlevania IV.
Masanori Oodachi = Masanori Oouchi + Masanori Adachi
Souji Taro = Akira Souji (aka akiropito) + Taro Kudou
Masanori Oouchi = sound effects
Akira Souji = sound program and a little bit of sound effects
Taro Kudou & Masanori Adachi = music

In the staff roll credits there is a credit for Sound Design: Souji Taro. Would that be you + Taro Kudou?- Since it cannot say in detail from me, I leave it to your imagination. :-)
Do you know if Masanori Oouchi also worked on Super Castlevania IV, or was it Masanori Adachi?- Yes. Mr.Ouchi is sound effects charge, Mr.Adachi is music charge. Both are my superiors together. :) Therefore, help was done.

My guess as to why their names were combined has to do with how the credits sequence is played out. Perhaps adding in two more entry screens to credit the sound team would've stretched it out too long, or there was some other discrepancy in that regard.
