Scans tell that it's from Shin Megami Tensei.

It's on Shin Megami Tensei Sound Collection. Disc 1, track 22.

EDIT: I was just about to add at least the game sources for each track to the notes, but then I noticed something: on the back cover, it says the "Hunting" track is from "Avatar Tuner", but on the back of the booklet it says "Avatar Tuner 2". Can someone confirm which one it's really from? Or is it actually from both maybe? And if so, shouldn't the products field have both games listed?

I would assume it's from both games. In Avatar Tuner 1&2 Integral there's a track called "Hunting" on disc 2 and disc 4. Discs 1 & 2 represent Avatar Tuner 1, Discs 1 & 2 = Avatar Tuner 2.
Plus there are other tracks on that album involving word "Hunting", but I'm not sure if they are some variations of the original or completely different compositions.
