On that page it says the CD was a pre-order bonus of PC version.
Release date for PC & PS2 versions in Japan was 23 Oct '03.
Also the publisher (manufacturer) for PC version on territory of Japan was Electornic Arts, as per the same Surugaya page and Amazon.co.jp. It's kinda strange, because PS2 version was published by Eidos, and both versions were released on the same day.
