Credited in the booklet of Akiko Shikata's album Harmonia as Wataru Hano: http://vgmdb.net/album/12723. His name only appears in English on this album, but the song he wrote the lyrics for is registered with JASRAC under his Japanese name.

Credited in the end credits of Ar tonelico Qoga as Syou Namino: http://www.youtube.com/watch?v=wkjJ8D7UDtI#t=4m21s. I did notice some name imperfectness with some other people (Akinobu "Mukaidani" instead of Mukaedani), so I'm currently favoring the Harmonia credit over this one.
