PSCX-1047: All Sounds of Vain Dream

A couple things about this:

First, why is Ikki Nakamura credited as "Abreath Nakamura?" The booklet just lists his name in kanji as 中村 一気.

Second, there's a confusing typo in the booklet regarding the arrangement credits. The credits are formatted like the following:

COMPOSER
1-8.25-40 Nobuhito Koise

Meaning he did tracks 1-8 and 25-40. The confusing part comes when we get to Tenpei Sato's arrange credits, which is written like this:

ARRANGE
21-21.40 Tenpei Sato

So... did he only do 21 and 40? Or is that second 21 supposed to be a 24? Also, Nobuhito Koise is also credited separately for track 21. Does this mean they both arranged it? Then why not credit it as Nobuhito Koise AND Tenpei Sato instead of crediting them both separately?
