Your best bet is to look for a gamerip. I've seen a rip from X360 version on the net, your "job" is to find it, I can't give a link to this kind of thing.

Tracklist has Mujula while cd shows Majula. So fixed that.

And back side of the CD has CA / - Dark Souls 2 - 53087 text on it, so could that incidate somekind of cat. #?

Can anybody confirm if the DLC tracks are by Motoi and/or Yuka? I've tried looking up let's plays on YouTube, but found no credits sequences.

Haven't had luck finding one either. Do the DLCs have own staff rolls at all this time?

Yeah, would be odd everyone being so meticulous omitting it if it were there. :P Maybe there will be one after the next/last DLC expansion. (Imo Yuka Kitamura definitely is involved there as well.)
