I'm not sure how you label "booklets" that are just one piece of paper with artwork on both sides. Insert seems the most appropriate since it's not a real booklet, but I've seen similar scans just titled "front" and "back".
Feel free to fix the note section, since I can't think of a good way to write the track summary, while including "actual sound source" and "bonus track".
I have yet to listen to the album, so I am unsure if the bonus tracks are only related to the preceding game (mostly because the last game is only one track).
