Default SECL-778: Rekishi Hiwa Historia Original Sound Track
A recent edit removed 'Kalafina' and added individual credits for the members. Is this how it is in the credits in the booklet?

Absent the booklet, I'm going to leave the performer as Kalafina.

I forgot i edited this entry. I have scans of the booklet, and that's how the vocalists are credited. Should i upload the scan somewhere?

Yes, you can upload them to the album by selecting "Manage Covers" under the edit menu.

Done. It seems Wakana Ootaki was credited as WAKANA. Which name should i use? Also, i'll ask a friend to transliterate the musician's names.

You can use WAKANA, it's listed as a variation for Wakana Ootaki so the system is able to link to her profile. I've linked up the artists.
