Lots of sites have album and artist info, but this one gives the members of 337めれ as (近藤達郎，大川俊司，小川美潮). I don't recognize any of those names though, so probably no one with a VGMdb pedigree.

The third track has a subtitle (as seen in the tracklist scan), but I can't find it listed anywhere and I'm not familiar enough with the kanji to type it up myself.

I'm not sure why this was listed as associated with the unreleased 64DD version. Looking over the scans I just uploaded it seems like it only mentions a CD-ROM release (though I can't read the text). My new cover scan (466667) is a lot darker than the current one (69066), but the cover really is that dark.

Maybe you have some kind of sunburn effect, but it should be lighter:
