The track order was wrong, fixed it based on both the Japanese and US version (which both happen to use the same order). Would be nice to have confirmation that this matches with the European version as well now.

Edit: Unlinked the tracklist for the European version, the previous track order turned out to be the correct one for that.

Found the scans for the Japanese version. (missing a disc scan)

So this is confirming all the CDs are the same and track orders are the same.

You didn't unlink the digital US version, the previous track list was correct for that.

A soundtrack CD is announced to come with the PC version of the game (24 Aug 2012).
The whole package will be called "Dark Souls: Prepare to Die Edition".
I don't know if the soundtrack CD will be any different from the past ones.

It's same as the on the European Limited Package, with the same tracklist mistakes.

Hmm, seems Takenouchi is credited in the game only as sound designer, with Sakuraba alone named for composition. Just goes to show how fluid these credits can be and how, unfortunately, we can't really be 100% confident in what the staff roll says being exact.

Props to TECHNOuchi for this nice piece (and linking the great arrangement of it). May have been the reason why it wasn't included on the OST release actually.

The role of "sound designer" may well include/involve that of an overall sound director (definitely the case here as there's no further sound related role in the credits), and it wouldn't be the first time one such slip in compositions without having them be explicitly credited.

Apparently the menu theme finally got an official release last year on the TECHNOuchi Encyclopedia 2007-2014, it's titled "Souls of Fire."
