Even though there is no official confirmation 瀬良みと = 山本希望, the fact that sprite updated the CV listings on the official site for the aokana game after the anime aired makes it pretty obvious that they are the same person. Feel free to unlink if you think otherwise (or if we don't link such aliases at all).

I've seen the same thing happen for other visual novels that received anime adaptations. It's official enough for us.
