I got a copy of this that I think is near complete (not sure if there was a registration card or not). I've uploaded images for all the pages of the booklet, a larger photo of the back of the case, the disc, the OBI, and the large poster that came with it.

On page 6 of the booklet it says that Kyoko Sakai and Ohito Takarabe handled the music for the first game and Hiroto Saito and Kyoko Sakai handled the music for the second one. I've added this information to the description. It does mention a few tracks by name and who composed them, but it's not very extensive so it may not be worth noting.
