If this is the Koji Yamada that worked at BEC from 1999 to 2011, then I'm pretty sure the artist's page should be linked, seeing as he's listed as a composer.

The second BGM guitarist is printed in the booklet as 尾崎啓一, but I do think it's probably a misprint for this Keiichi Ozaki (尾嵜 啓一): http://kei-oz.com/. I couldn't find any record of him working with the composers or other performers on anything else, or else I would've said I'm definitely sure.

The vocalists 早川由佳子 and 富岡由紀子 are probably Yukako Hayakawa and Yukiko Tomioka, but they seem to be a bit obscure as far as VAs go and I could only find somewhat dubious resources giving readings for their names.

The readings are correct. I went onto Nico Nico Douga and watched the game's credits via a Japanese Let's Play: http://www.nicovideo.jp/watch/sm2386...arch_key_video

The credits are about 18-19 minutes in. You need a Nico Nico acct, but they aren't too hard to make. I have attached screenshots for proof positive of romanization. This is from the credits attached to the game itself.
