It should be noted that the Bluray is broken up into 2 chapters for the bonus Sapporo Kitara Hall performance and 26 chapters for main concert. There are 10 additional tracks in the main concert that aren't technically part of the tracklist but are part of the chapter index.

Title index:

title00 -- This is the main menu screen with just a fade of Koichi and the concert in the background and it plays 勇者の仲間たち (Comrades) on loop
title01 -- This is the bonus content. 2 chapters, 1 for each track.
title02 -- This is the main concert, it is broken down here by chapter:

01 Intro 0:15 -- silence, displays the "Don't sit too close to your TV" warning. Starts with: "鑑賞上の注意
で視視聴の際は部屋を明るくし、できるだけ画面より" and I didn't bother inputting anything past that.

Products represented: Dragon Quest X as well, according to the notes.

When we exclude the bonus tracks from X, are there any new ones compared to the previous Symphonic Suites of V? http://vgmdb.net/album/1630 http://vgmdb.net/album/2918

About the track list on the album page, not dancey's post above:
14 seems to be from IX, 15 from II, 16 from III, 18 from X, and 17 may be from X as well if the note is correct in saying there are two bonus tracks from X.
4 seems to be new.
