don't know whence the tracklist comes but there are 2 errors
14 環状氷山フォロスヒルデ -> 14 環状氷山フロスヒルデ
17 グレイブニルの縛鎖 -> グレイプニルの縛鎖
as seen on some scan where the tracklist is written

even without scan, those errors are easily visible since gleipnir is the name of the chains that bind fenrir till ragnarok
and woglinde/wellgunde/flosshilde are three nymphs from the famous opera of wagner

also, I watched credits for the localized game, and it has 6 names for Sound and 1 name for Main Theme Music (no idea what it is in the game)
6 composers? no idea if there is music from other sao games in this game

Tokusashi was always credited also in the previous games for "Main Theme" but at least to me it was never clear if that was included on the soundtrack or not, which is why he's only mentioned in the notes on the other albums. See this: http://vgmdb.net/forums/showthread.php?t=14041

What are the other names in the English credits?

It says Ryuji Nishida, Shuhei Seki, Tomoya Sato, Kouhei Takahashi, Hiroyuki Maruyama, Kazuma Ishii.

Uh ... so I guess none of those is the 藤枝亮平 that was credited for Music in the previous games.
Not really sure if this release is complete and who should be credited here honestly.

There is Sound: Ryouhei Fujieda on Hollow Fragment credits. I don't know if their reading is accurate.

I don't think it would be read any other way.
