I think the name of scans are quite confusing, though I can't use more proper words to name the scans.

First, that "Tray" is not a tray. I didn't scan the tray pictures because they are sticked with the plastic trays. The entire paper case is folded, and the "Back" is the back of disc 1 tray and that so-called "Tray (cropped)" is actually the back of disc 2 tray. The cover is 2-layer, I didn't scan the cover back cause there is no information on it. The folded booklet is inserted into this 2-layer cover.

Oh. I am agree it's hard to find adequate names for scans.
I thought it was a folded booklet of 4 pages large, which folds into 2, and 2.
On the example bellow: "Double Gate Fold"

We can use "Disc 1 Tray", "Disc 2 Tray". booklet should be renamed if it's not like i thought.

Exciting update for this one! I added some previously unknown track-by-track composition credits for Naoto Ishikawa and Ryu Umemoto (who are only credited for Production Assistance [制作協力] in the booklet). The information came directly from Hideki Higuchi via Twitter direct message.
