The sleeve and the disc of this release are stating the track-list as follows:

The point of interest is "（全5話）", which can be roughly translated as 5 episodes(?). It is not clear whether this means that they are combined (2 tracks in total), or if they are separated into 5 tracks (6 tracks in total). Therefore, I don't want to add anything until somebody with the disc in hand can confirm it.
In regards to the credits - the disc only states the VA names and not the character names. All the other credits are located on the sleeve, and are related to the blu-ray discs only.
