ZTTL-0037: Psychic Force 2012 Arrange sound tracks

There are performance credits on the bottom of the Booklet 5 scan (middle row), but I can't make them out really well. They look like:

Guitar: Fumito Machida (1), Tomohito Takahashi (6?)
Sax: Takashi Koda? (Zuntata) (6?)

I'm reasonably sure about Fumito Machida and track 1, as well as Tomohito Takahashi's name, but the other track numbers and the name of the sax player I'm not confident about.

Actually, it's probably Takashi Ikeda, who performed sax on ZTTL-0044.
