I think she's the same person as Tomoko Kobayashi and would have married Hirotomi Imoto sometime between 2004 and 2005.

As her profile says she joined Sacnoth in 2001 and transferred to Nautilus later, she must have been involved in the second Shadow Hearts title unless she did nothing until SH:FTNW, but the only staff with "Tomoko" in the game credit is Tomoko Kobayashi (although the credit is incomplete in that it lacks her involvement in the composing role).

I know it still may not be evident enough, though.

This is correct. The game credits show she has also maintained a similar role between the Shadow Hearts II and III by handling battle sound effects on both and battle composition on the latter.

For those who are interested, these days she works as a sound designer with feelplus.
