Track 19 of this media is written as 落ち葉の栞, and a very simillar track appears in SRCL-4457~8 and KDSD-10005~6 but with the name 落葉の栞.

It's probably the same track. The readings of those two would be identical. A lot of compound words with verb stems can omit the kana attached to the stem and still be read the same way:

立ち入り vs. 立入
飛び出し vs. 飛出し
etc.

There were four different versions of that track, featuring at least two different ways of spelling across their titles. I haven't heard this CD, so I wouldn't know which one this is.

Well, I have heard them and they seem to be the same tracks (at least start identically and the modbar for Clementine are identical for 落ち葉の栞 and 落葉の栞 -of KDSD-10005-)

But we better have someone, like a trusted editor, confirm it.
