The composer for the game Tentana is listed as Makoto Yamamoto on GMCL.

Are you sure the tracklist is the good one on vgmdb for this album?

I still have the original tracklist (from Teioh), and it does not correspond at all with vgmdb.
It's more like this:
Tentama Sound Collection +Alpha japanese tracklist

Any informations/corrections?

It looks like the tracklist on JGML is wrong, which was the source for that one. The link you gave is more plausible, since the Synth and Non-Synth versions line up, so I retranslated using that link and replaced the old ones.

Are we still unsure of the reading for the composer's name? Is their first name definitely まこと (Makoto)? Could it be しんじ (Shinji)?

Also, there were two pages missing from the scans. The scan for pages 05-06 was mislabeled as pages 03-04. I've corrected the name of that scan and uploaded a correct image for pages 03-04. I also added images for the disc and back of the case.
