Added larger covers.
Also added scans of the inside and the disc. but im having problems uploading the Back scan (res: 4076x2147) says file too big should I diminish the quality ?
Also could a mod delete the smaller scans. thx

Try to get it slightly below 5 MB, there should be no need to change resolution.

ok didnt change the res but had to change from JPEG High to JPEG Medium with Photoshop
Edit: also thx Myrkul for editing the Disc image. If somebody could remove the smaller res Back pic that would be nice thx

the "remix" selected is for that ff1 track? it shouldn't be prototype/unused?

My copy of this album includes an insert, which strangely, seems to advertise Final Fantasy IV on the front and back.

Because the insert doesn't contain the catalog number of the album, and because I can't see the insert pictured in other copies of this album for sale, I'm not 100% it originally came with this albumy. Though the insert does say "NTT" on it, and is too long to fit in a jewel case, so it almost certainly came with one of these FF 8cm singles.

Would anyone be able to confirm whether their copy also has this insert? If it does, we can include it in the gallery.

In terms of whether this actually came with the album, here are some reasons to think that it did:

- The insert bears the same name as the publisher of the album (NTT)
- At least one person had it included with the album they bought (i.e., me)
- The insert is too large to have fit in an ordinary jewel case. If it came with an album, it must have been an 8 cm single
- Some other albums in this "series" came with an extra (e.g. Final Fantasy IV Minimum Album and Final Fantasy VI Special Tracks)

and reasons to think it didn't:

- The game mentioned on the album (FF IV) is different from the game the album is from (FF V).
- So far, only one person reports to have the insert, out of all who own the album on vgmdb.

Thanks for the scans Rrolack, it's indeed an odd one.
FF4 SFC release was on July 19, 1991.
The earliest 8cm single was FFIV Minimum album, released on Sep 5th 1991.
From the message on the insert "two FFs coming this summer", this couldn't have been provided even with FFIV Minimum, let alone this FFV+1.
What's mysterious is the size of the insert, does it fit exactly in a 8cm casing?

It doesn't fit an 8cm casing exactly, but it's far too large to fit in a standard jewel case.

It advertises FFIV and Seiken Densetsu: Final Fantasy Gaiden, which were both released in summer 1991 (June and July).
Maybe it was included with one of the soundtracks from those games? They were released around the same time as the games and the original versions were digipaks so they're slightly larger than jewel cases.

Just ripped this one but couldn't look up the album with freedb. Looks like track 4 is off by a minute (1:30 -> 2:30) so it's fixed and now lookupable. Having said that, my rip has +1 second on all six tracks for a total of 6 seconds.

One second difference is usually not a real difference per se, just different hardware reading things slightly different.
