Not sure where the catalogue number of KTGS-40312CD-1~2 came from (it seems like we're the only source), but according to the disc scans it should be KTGS-79689CD-1~2.

KTGS-30312 is PS3 treasure box
KTGS-40314 is PS4 treasure box
I guess koei used KTGS-79689CD for both releases (a la KCS-xxxx for other releases)
not to make a printing with KTGS-30312CD-1/2 and another with KTGS-40314CD-1/2
