Has Takenouchi's original megadrive soundtrack for the game ever been published in CD form?

Not to my knowledge. Not even individual pieces on compilation albums.

Well, isn't that a shame... Sometimes I'm really shocked at how many important works didn't find their way on a disc.

As I suspected long ago, the romanized name form of 八木秀之 is Hideyuki Yagi.

This source is the first one that I find where it gives his romanized form. There are more sources (such as this or this other) where he is always mentioned as a flute teacher and that sometimes performs in concerts of classical repertoire.

Also, the mixing engineer that wasn't romanized until now is Yoko Uehara. Credited in this album of the same year and under the same record company (Scrumm Staff). Also credited in our database (here and here too).

I added track credits to each performer based on what I hear. The strings are a bit tricky because on track 1 and 12 the string trio could have conceivably been spliced with the synth string orchestra. It's hard to tell. I'm going to say they are there.

I wonder how Takenouchi decided when and where to best employ the players? All instruments are actually featured prominently throughout the score, leading me to believe it was originally intended that ALL parts be recorded live. It's a masterpiece either way.
