there's a blue card too, though i have no idea what those cards actually are. they don't look like slipcases, so maybe they are cut-out cards to insert inside your jewel case?

It's part of a page you'd cut out from the magazine to fit in your own jewel case, same as the other Tokuma Shoten magazine bonuses. The owner of that one cut theirs into two separate covers but you can leave it in one piece and fold it one way for one cover or the other way for the alternate, which is how I've found most of them.
