Default Wave Race: Blue Storm Music CD
I believe that this was a contest prize for Nintendo Power magazine. I'll let someone who knows for sure add it to the notes.

Was able to find archive.org has some issues preserved and add picture. Also found that the issue was from 2002 and so made edit to release date.
