Naoya Kamisaka's profile on his old site Sound Avenue says that Kamisaka is just a pen name, and his real name is 古谷直也. We can cross-reference that with the credits of Boulder Dash EX (which he collaborated on with Soramimi Studio owner Yasuhiro Inazawa) to get the reading as Naoya Furutani. I don't think he ever appeared on an album under his real name.
