This album currently credits "Eiji Suzuki, Yoaki Takahashi, Yutaka Fujishima" for composition, but:

• "Eiji Suzuki" should probably be Hideyuki Suzuki (鈴木 英之) and "Yoaki Takahashi" should probably be Hiroaki Takahashi, and
• The game credits only H. Suzuki as the "sound composer," with H. Takahashi and Yutaka Fujishima doing sound effects.

So I'm curious about if and how anyone is actually credited for music in the booklet.

You were right about all three guys, they are all credited in the booklet. However, I think the game credits have the most accurate breakdown with Suzuki doing the composition and Takahashi and Fujishima doing the sound effects.
