Hopefully the booklet will have a full list of performers (if there are any others), but in the meantime, TAM's confirmed that he performed violin for this soundtrack. He even posted a teaser video with a tune and a picture of him with Shinji Orito: http://www.youtube.com/watch?v=1LV4zndXsuQ.

This is also the first time he's performed for a game!

Will this be available to order outside of C80 eventually?

I really hope it will (although I fear the worst). And while they're at it, please also do an official release of Albina and the Kud Wafter soundtrack.

The soundtrack will receive a regular release on October 28th. Rakuten already seems to accept orders, I'm sure Amazon and CDJapan will follow.

I found composition credits in... uh... somewhere (tags). I can't vouch for their accuracy, so I'm putting them here:

Jun Maeda: 102
Ryo Mizutsuki: 103 106 107 110 207 208 214 220
Shinji Orito: 104 111 112 114 116 120 121 202 204 205 206 218 221 222 223 303 317 318
Maiko Iuchi: 105 109 113 115 118 203 209 210 211 217 311
Soshi Hosoi: 108 117 119 212 213 215 216 219 304 305

For what it's worth, those credits match up with those on the Rewrite soundtrack wikipedia page. That page also has details on the arrangers of each track, though I can't account for its validity.

Scans added, hopefully they'll have useful information for you.

The artist credited as 井出 浩基 on the booklet is the same that the one credited on this album as 出井 浩基.

I don't know which one is the correct one, but at least one of the possible readings of 井出浩基 could be "Hiroki Ide" and someone with that name is the drummer of a band called TIG. Unfortunately, there is zero information about this artist in japanese, so I can't verify anything about him.

Added a Romaji track-list - Used the Wikipedia entry as a base and then the booklet to keep it as close to the source as possible.

In regards to D-03;M-17~18, pages 02-03 of the booklet do not credit the composition and arrangement. However, the liner notes include names next to each track title, which also happen to be the composers of each of the tracks on the soundtrack. I originally thought that it was there to indicate the person who wrote the notes, but I'm not so sure now because I cannot translate them. If it is not for this purpose and it is the case that they are in fact the composers, then Shinji Orito would have composed those last two tracks. I have not added this in the notes, but I will leave it here as a theory until it can be confirmed.

The notes are just general comments (~ both are treated as SFX in game; 3.17 was modelled from the main part of 1.02 "Travel", reversed).

But some other track notes do list who wrote the comment along with the composer (ex. 3.11), unlike 3.17/18, so I'd think Shinji Orito wrote them.
