It looks like an advertisement flyer, so if it did originally come with this release it probably came with all copies.

Oh my goodness, today Masami Ueda himself sent me back an e-mail reply with composer breakdowns for both Biohazard 2 and 3.

They're on the entries for the soundtracks right now, so go check them out!

This also reveals that Naoshi Mizuta actually composed a track on the soundtrack release of Biohazard 2 as well.

Does the dog tag have 5 digits 00000 or is that D0000 ?
Anybody have an idea how many were actually made? could be 1k, 10k, 99k...

Pretty sure it's five digits. Here's one that indicates at least 20000 copies (think it says 19557?):

Just picked up the copy I wanted, it is indeed five digits, mine is 19614. I wonder they stopped around 20000?

This album came with a black tray? Mine has a transparent tray and it doesn't close fully, pretty sure it was replaced by the previous owner. Hate when they do this.

Yes, my sealed copy has a black tray.
