There's a mention of it on STC: "the 3-song disc that kMart gave with Ocarina of Time..."

Other than that, I dug around on freedbtest and found this 4-track CD. There's a VGMdb entry with apparently the same 4 tracks but with german titles, and it was a Europe promo disc.

Came with a shirt, packed, sealed and attached to the album.

Actually it's not really rare, you can still get the CD at eBay Germany from time to time. Worth around 3-6 Euros.
Edit: I refer to the german one for sure.
