Default PRT-8429: EYES ON ME: featured in FINAL FANTASY VIII
All the information I could find on this one is from a YouTube video:

Apparently it is a rare Japanese vinyl promo, so it might be pretty hard to find more information on it. I'm not even sure about the actual name of this promo, actually.

Discogs is a good resource for vinyls i think so i always check there for some infos & maybe some pictures.
