Default LOTUS CD 003: Sorairo no Organ VOCAL COLLECTION
I added images for this one: booklet (front, and all pages), disc (front and reverse), back. There's nothing on the tray.

The booklet unfolds into one accordion folded piece of paper. The order the lyrics are printed isn't the same order as how they appear on the CD. I wasn't sure how to label the page numbers, so I just went in order from left to right starting with one side, then continued from left to right after flipping over to the other side.
