Well, the game was released on September 30, 1999, and it seems the disc was included within the package.

I am pretty sure it's a duplicate..
For the date, wiki's i have consulted are all agree with the following date:
5 october 1999.

edit: gamefaqs have 30 september..

oh well, just pick one

It's the same CD, packaged with the FF Anthology release for the PSX. It just seems to be a case of confusion as to the exact release date. But Square Enix confirms the release date as October 5th (link). And DIDX-068128 3 is written on the back of the cd (inner rim).

Nice work on figuring this out. I would have been surprised if there were a major FF release that we'd missed. It looks like Gigablah is moving the collection and ratings to the original.

Since I was poking around a bit for info on the DIDX stem, I might as well post this here:

I deleted the old disc scan as the new one is both bigger and has correct colors.

FWIW, DIDX-068128 3 should not be used as a catalog number, as it has nothing to do with Squaresoft. It's a pressing plant ID number (Sony DADC USA) of the master. Usually those numbers are only written in the matrix code, but for some reason DADC often print it on the label, too (oh, it's not on the disc label on this disc, so I guess you found it in the matrix code. Should be careful when you do that, the numbers may have nothing to do with the actual release).
