Dunno if you want a full translation of the insert, but if a summary is fine it's basically just congratulating you for winning the prize lottery and thanking you for supporting Natsume over the years.

Another thing I'm curious about is whether all copies of this are sample discs. My copy is, and the picture of the case back that was uploaded previously also showed a sample copy. Since it was a prize and not sold at retail, I suppose it's possible, but I could be wrong.
